In this interview, we take a detailed look at the nuances of open and closed rhinoplasty, as well as facelift and breast augmentation.

Dr. Engin Ocal is a Turkish plastic surgeon specializing in rhinoplasty, deep SMAS facelift, and breast augmentation. — With open rhinoplasty, the final result can be expected after 12 months. After how long can the effect of closed rhinoplasty be assessed?

— Recovery after closed rhinoplasty largely depends on skin thickness. Patients with thin skin recover faster, and those with thick skin somewhat longer. The final result of the surgery can be seen after 3–4 months.

— Is closed rhinoplasty suitable for patients with breathing problems or those who need septoplasty?

— With the closed rhinoplasty technique, all the same manipulations can be performed as in open surgery — there is no difference here. However, if a patient needs revision (repeat) rhinoplasty, I prefer to perform the operation using the open approach.

— Tell us how you discuss the final result of the surgery with the patient.

— For a good aesthetic result, it is important to consider not only the patient’s wishes but also their anatomical features. Most patients cannot assess on their own which option would suit them best.

When a patient comes to us wanting rhinoplasty, we discuss what type of nose she likes. Some people like a straight nose, some an upturned one, and some a natural look. Sometimes the internal septum also needs to be corrected. First, I create a 2D reconstruction of the nose and discuss with the patient which option is best for her, and then I give the nose its final shape using 3D modeling. After that, I perform the surgery, following the computer’s calculations. This approach allows us to achieve excellent results.

— So the patient can understand, even before the surgery, what their nose will look like afterward?

— With the Vectra system, the patient can imagine what the new shape of the nose will be. It is not a 100 % prediction, but it reflects roughly 85 % of how the new nose will look.

— You mentioned that you specialize in facelifts. Could you tell us a little about this procedure — how you perform it and what result to expect?

— We usually perform a deep SMAS lift. This means we tighten not only the skin but also the superficial musculoaponeurotic system. If you tighten only the upper layer of skin, in 3–4 years it will sag again and unsightly scars will become visible. That is exactly why we prefer a deep facelift.

Beneath the skin lies strong SMAS tissue. When the surgeon tightens it during surgery and attaches it to firm areas of the skull bones, it is securely fixed in its new position. That is why the result of such a procedure can last 15 or 20 years.

A deep facelift, especially in women, makes it possible to achieve a beautifully defined contour of the chin and neck. It also allows the area under the eyes to be tightened well. That is why our patients are very satisfied with the result of this surgery.

— Let’s move on to breast augmentation. How do you perform it, what technique do you use, and which implants do you use?

— For primary breast augmentation, when I discuss the upcoming surgery with a patient, I ask what breast size she likes and what size she would like to have. Sometimes patients send me their own photos or photos of other women to explain their preferences. Then we discuss the matter again using 3D modeling to determine the optimal size given the type of breast and the volume of the chest. Importantly, we always adhere to strict medical standards calculated by computer, so the result of the surgery can be predicted in advance.

As for implants, I mainly use Polytech — a German brand that I trust. I like the architecture of these Polytech implants and of the new-generation implants from other brands, such as Mentor. They are truly good.

In addition, it is important that the surgery be performed cleanly, meaning without microbial contamination. This minimizes the risk of capsular contracture developing. This was the topic of my doctoral dissertation, so I have studied the matter thoroughly.

— Is there a risk that the body will not accept the implant? Some patients fear that something will happen with the implant — that inflammation or rejection will begin.

— Yes, this does happen occasionally, but it is rare: we have not encountered such problems. Sometimes patients psychologically do not accept their new breast shape — not because of immunity or physiological factors, but precisely because of psychological aspects. In such cases, the implant can be safely removed. New breast augmentation techniques, such as placing the implant under the muscle tissue, help avoid problems in the future. Over the past ten years, we have not encountered serious complications in our patients. We approach surgical planning carefully, work in prestigious clinics, and use high-quality materials. That is why we receive good reviews of our work, and I like that.
